Primary health care is essential health care that is accessible to individuals and families in the community so that they are able to receive the services they need when and where they need them.
At Hamilton Urban Core, services and programs including primary health care, are provided by multidisciplinary and inter-professional teams of highly qualified health providers. Working collaboratively, the health care team addresses not only physical health needs but also considers how health promotion, illness prevention, the social determinants of health and community well-being all relate to the overall health of individuals and families.
Services include treatment of episodic illness, chronic disease management, chronic pain management, well-person care, health teaching and education, counseling, and making referrals.
The Primary Health Care Team at Hamilton Urban Core includes family practice physicians, nurse practitioners, primary care nurses, and a physician assistant.
